Telemedicine app development in India – video consultation, digital prescription and online payments
Booking, video consultation, prescription and payment – one secure platform.

Key takeaways

  • A telemedicine platform is more than video: booking, intake forms, prescriptions, payments and follow-ups must work together.
  • Structured patient analysis forms save consultation time and improve diagnosis quality.
  • Role-based dashboards for admin, doctor and patient keep the experience simple and data secure.
  • Follow India's Telemedicine Practice Guidelines (2020) and the Digital Personal Data Protection Act, 2023.
  • Start with web (no install needed) and add Android/iOS apps when patient volume grows.

Since 2020, online consultation has become normal in India – especially for follow-ups, chronic care and patients who live far from the doctor. But a WhatsApp video call and a UPI screenshot is not a system. A proper telemedicine platform handles booking, the consultation, the prescription, the payment and the follow-up – securely.

We built Jivantanir, a homeopathic teleconsultation platform, at RelieveX Labs. This guide explains the modules you need, the decisions that matter and the rules to follow in India.

Who needs a telemedicine platform?

  • Doctors and clinics with patients in other cities or villages
  • Homeopathy, Ayurveda and lifestyle practices with long treatment courses
  • Specialists offering second opinions
  • Hospitals that want structured follow-up consultations

The core modules

1. Doctor profiles and online booking

Patients choose a doctor, see available time slots and book. Slot management prevents double bookings and no-shows can be reduced with reminders.

2. Payments

We integrate Razorpay so patients can pay by UPI, card or net banking and the booking is confirmed automatically – no screenshots to check.

3. Smart patient analysis form

Before the call, the patient fills a structured intake form – symptoms, history, current medicines. For homeopathy this is especially detailed. The doctor starts the consultation already informed, which saves time and improves accuracy.

4. Secure real-time video

Jivantanir uses a custom real-time video system built on WebRTC with Socket.IO signalling. Calls run in the browser – patients do not need to install anything.

5. Digital prescriptions and records

The doctor writes the prescription in the platform; the patient receives it instantly and it stays in their history for the next consultation.

6. Follow-ups and treatment tracking

Long treatments need follow-up reminders and a clear history of every consultation.

7. Role-based dashboards

Separate dashboards for admin, doctor and patient keep each experience simple and data access controlled.

Rules and privacy you must respect

In India, registered medical practitioners can offer teleconsultation under the Telemedicine Practice Guidelines released in 2020, and AYUSH practitioners follow their own council's rules. Your platform should support identity verification, patient consent and proper record keeping.

Patient data is personal data under the Digital Personal Data Protection Act, 2023. Practical steps:

  • HTTPS and secure authentication (JWT with short sessions)
  • Role-based access control
  • Secure file storage for reports and prescriptions
  • Collect only necessary data; publish a clear privacy policy
  • Offer account and data deletion

Web first or app first?

For most practices we recommend web first: patients click a link and join, nothing to install. When volume grows, add Android and iOS apps from the same backend for reminders and quick access. If your practice also needs in-clinic records and visit tracking, see how we approached it in physiotherapy clinic management software and the DVM app.

Results from Jivantanir

Patients from different locations can now consult without travelling, doctors work faster with structured forms and automatic payments, and medical records are organised and secure – reducing dependence on physical clinic visits.

Cost and timeline

PhaseIncludesTypical time
Phase 1 – web platformBooking, payments, intake form, video, prescriptions, dashboards8–12 weeks
Phase 2 – appsAndroid & iOS apps, push reminders4–8 weeks
Phase 3 – growthMulti-clinic, pharmacy, analyticsas needed

A web platform typically starts from about ₹2,50,000; see our cost guide for how features change the price.

Choosing the video technology

There are two main options for consultation video:

  • Custom WebRTC – the browser technology behind most video calls. Full control over the experience and no per-minute fees, but it needs expertise in signalling, TURN servers and poor-network handling. Jivantanir uses this approach.
  • Video SDK providers – faster to integrate, with recording and analytics built in, but billed per participant-minute. Good for pilots or very high volumes.

Either way, test on real Indian networks: 4G in small towns, older Android phones and low light. A consultation that drops every two minutes destroys trust.

The patient experience checklist

  1. Book in under two minutes, in the patient's language where possible.
  2. Pay by UPI with automatic confirmation.
  3. Receive reminders before the consultation.
  4. Join from a link – no app install required.
  5. Get the prescription and advice immediately after the call.
  6. Book a follow-up in one tap.

Growing beyond one doctor

As practices grow, the platform should support multiple doctors with their own schedules and fees, clinic-level reports, discount codes for camps or corporate tie-ups, and integration with pharmacies or diagnostic labs. Designing roles and permissions properly from day one makes this growth straightforward.

Frequently asked questions

What features does a telemedicine app need?

At minimum: doctor profiles, slot booking, online payment, secure video consultation, a patient intake or analysis form, digital prescriptions, consultation history and follow-up reminders – with separate dashboards for patients, doctors and admins.

Is online consultation legal in India?

Yes. Registered medical practitioners can consult online under the Telemedicine Practice Guidelines released in 2020, and AYUSH practitioners follow the rules of their councils. The platform should help doctors follow these rules – identity, consent and proper records.

How much does a telemedicine platform cost to build?

A web-based platform with booking, video, prescriptions and payments typically starts from about ₹2,50,000; adding Android and iOS apps, multiple clinics or pharmacy integration increases the cost. Building in phases keeps the first launch affordable.

Which payment gateway is best for doctor consultations?

Razorpay is widely used in India and supports UPI, cards, net banking and wallets with automatic payment confirmation. Fees are charged per transaction – check the current pricing on Razorpay's website.

How is patient data kept secure?

Use HTTPS everywhere, secure authentication, role-based access, encrypted storage for files, careful logging and regular backups – and collect only the data you need, as required by the DPDP Act, 2023.
